HIPAA Notice of Privacy Practices

Likeminds Behavioral LLC

Effective Date: October 18, 2026

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Your Information. Your Rights. Our Responsibilities.

Likeminds Behavioral LLC is committed to protecting the privacy of your health information. This Notice of Privacy Practices explains how your protected health information may be used or disclosed, your rights regarding that information, and our legal responsibilities.

This notice applies to protected health information maintained by Likeminds Behavioral LLC to the extent the organization is subject to HIPAA and other applicable privacy laws.

1. Your Rights Regarding Your Health Information

When HIPAA applies, you have certain rights concerning your protected health information.

Right to Access Your Health Records

You may request to inspect or obtain a copy of your health records and other information we maintain about you, subject to applicable legal exceptions.

We may require your request to be submitted in writing. Where permitted by law, reasonable fees may apply for copies.

Right to Request Corrections

If you believe information in your health record is incorrect or incomplete, you may request an amendment.

We may deny a request in certain circumstances permitted by law. If we deny your request, we will provide the required explanation and information about your available options.

Right to Request Confidential Communications

You may ask us to contact you through a particular method or at a specific location.

For example, you may request that we contact you at a designated telephone number or mailing address. We will accommodate reasonable requests as required by applicable law.

Right to Request Restrictions

You may ask us to limit how we use or disclose certain health information for treatment, payment, or healthcare operations.

We are generally not required to agree to every requested restriction. However, HIPAA requires certain restrictions in specific circumstances, including qualifying requests not to disclose information to a health plan when the services have been paid for in full out of pocket.

Right to Request an Accounting of Disclosures

You may request a list of certain disclosures of your health information made during the period allowed by law.

This accounting does not include every disclosure. For example, certain disclosures for treatment, payment, healthcare operations, or disclosures made with your authorization may be excluded.

Right to Obtain a Copy of This Notice

You may request a paper copy of this Notice of Privacy Practices at any time, even if you have agreed to receive it electronically.

Right to Choose Someone to Act for You

If someone has legal authority to act on your behalf, such as a legally authorized personal representative, that person may exercise applicable privacy rights for you, subject to the requirements of law.

Right to File a Complaint

You may file a complaint with Likeminds Behavioral LLC or the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.

We will not retaliate against you for filing a complaint.

2. How We May Use and Disclose Your Health Information

We may use or disclose protected health information without your written authorization in circumstances permitted or required by law.

For Treatment

We may use and share relevant health information to provide, coordinate, or manage your behavioral healthcare.

For example, information may be shared with another healthcare professional involved in your care when the disclosure is permitted by applicable law.

For Payment

We may use or disclose relevant information to obtain payment for healthcare services.

For example, we may provide information to an insurance plan to determine coverage or process a claim, where applicable.

For Healthcare Operations

We may use information for activities needed to operate our healthcare practice, such as quality assessment, administrative functions, compliance activities, and service improvement.

For example, authorized personnel may review records when necessary to assess the quality of care or support appropriate business operations.

With Business Associates

We may work with service providers that perform functions on our behalf, such as secure electronic record storage, billing, or technology support.

When a service provider qualifies as a HIPAA business associate, we require appropriate contractual safeguards and protections for the health information it handles.

When Required by Law

We may disclose health information when federal, state, or other applicable law requires us to do so.

For Public Health and Safety

Where permitted by law, we may disclose information for public health activities, certain reporting requirements, or to help prevent a serious and imminent threat to health or safety.

For Health Oversight

We may disclose information to authorized government agencies for legally permitted audits, investigations, inspections, or other health oversight activities.

For Judicial and Administrative Proceedings

We may disclose information in response to a court or administrative order or certain legal processes when the disclosure is permitted by applicable law.

For Law Enforcement and Other Government Functions

We may disclose information to law enforcement or authorized government officials in specific circumstances permitted or required by law.

For Research, Organ and Tissue Donation, and Other Permitted Purposes

Certain disclosures may be permitted for approved research, organ or tissue donation, workers' compensation, and other legally recognized purposes, subject to applicable safeguards and restrictions.

3. Uses and Disclosures That Require Your Written Authorization

Certain uses and disclosures generally require your written authorization, including uses or disclosures that are not otherwise permitted or required by law.

Examples may include:

  • Certain uses and disclosures of psychotherapy notes.

  • Marketing activities that require authorization.

  • Disclosures of health information for purposes not otherwise permitted by law.

If you provide an authorization, you may revoke it in writing, subject to applicable legal limitations and actions already taken in reliance on that authorization.

We will not use or disclose your health information for a purpose requiring authorization unless the required authorization has been obtained.

4. Special Protections for Certain Health Information

Some types of health information may receive additional protection under federal or state law.

These protections may include certain mental health records, psychotherapy notes, substance use disorder treatment records, and other specially protected information.

Where federal substance use disorder confidentiality requirements under 42 CFR Part 2 apply, we will follow the additional requirements governing the use and disclosure of those records. These rules may require specific consent for certain disclosures and provide additional protection against the use of records in legal proceedings against a patient.

The exact requirements depend on the type of record, the services provided, and the laws applicable to the organization.

5. Our Responsibilities

Where HIPAA applies, Likeminds Behavioral LLC is required to:

  • Maintain the privacy and security of protected health information.

  • Provide patients with notice of our legal duties and privacy practices.

  • Follow the terms of the Notice currently in effect.

  • Notify affected individuals following a breach of unsecured protected health information when notification is required by law.

  • Limit uses and disclosures to those permitted or required by applicable law.

  • Follow additional privacy protections where more stringent laws apply.

We reserve the right to revise this Notice when our privacy practices or legal requirements change. Revised terms may apply to information we already maintain when permitted by law.

Any updated notice will be made available as required, including through our website and at our office locations, where applicable.

6. Maryland and Other Applicable Privacy Laws

In addition to federal requirements, our handling of health information may be subject to applicable state privacy and medical-record confidentiality laws.

The laws that apply may depend on the services provided, the location of care, professional licensing, the type of information involved, and other relevant circumstances.

Where applicable law provides greater privacy protection than the general HIPAA standard, we will follow the requirements that apply to the information and circumstances involved.

7. How to File a Privacy Complaint

If you believe your privacy rights have been violated, you may contact our practice or submit a complaint to the U.S. Department of Health and Human Services, Office for Civil Rights.
